Form 355 (Affiliated Entity Disclosure) must be completed under any of the following conditions: * A director of the firm owns part of another business or firm * An officer of the company owns part of another business or firm * A shareholder of greater than 10% owns part of another business or firm * A close relative, meaning brother, sister, parent or child of the owner of the 8(a) firm owns another business or firm
